Policies
- If the need arises and you are unable to keep your scheduled appointment, a 24 hour notice is required. You will be charged a $25.00 fee if you do not call us to cancel.
- When planning your time, please arrive 20 minutes prior to your scheduled appointment to enable you to update any necessary information and be prepared to see your physician.
- If your insurance company requires a co-pay for your visits, that co-pay is collected when checking in at the front desk.
- Please bring your insurance card(s) with you to all appointments. We will submit claims on your behalf, providing you give us permission to do so. This is done by signing our HIPAA notice of privacy practices at the time of your initial visit to our office.
- You will be billed for any amount not covered by your policy. We accept most insurances, PPO’s, and Medicare. We do not accept any HMO’s. It is always the patients’ responsibility to verify that your insurance is accepted at our office and laboratory.
- Labs, x-ray reports, and other patient related information will be shared with you by your physician or his/her designee. Please consult your physician for details about his/her process.
- If you should require hopitalization, you will be seen by any one of the highly skilled hospitalists during the course of your inpatient stay.
